OFFICIAL TRAVEL AGENT

JTB Corp. (JTB), appointed as the official travel agent for the Assembly, will handle hotel accommodations and provide exclusive offer of various attractive tours to the participants.

1. Hotel accommodations

JTB has blocked rooms at hotels in Sapporo for the Assembly period. Reservations will be made in order of receipt of the application form. If the hotel of your first choice is fully booked, JTB will reserve your second choice or a hotel in the same grade. Prices are given in Japanese Yen.

Notes:

• Room rates include service charge. A 5% tax will be added to your bill.

2. Public Guest Houses

Accommodation at Public Guest Houses is available at relatively low rates.
Guests of these facilities will be asked to follow the basic rules of each facility.

Participants wishing to reserve hotel accommodation should complete the Application Form and return it by fax or by mail to reach JTB no later than April 30, 2003 (Confirmation will be sent by JTB).
The application should be accompanied by a remittance covering the hotel deposit (one night room charge or total room charge) due JTB. (The hotel deposit will be credited to your bill. All hotel expenses decucting the hotel deposit should be paid directly to the hotel.)
No reservation will be confirmed in the absence of this payment. Personal checks are NOT accepted. All payment must be in Japanese yen. The remitter's name should be the same as the participant's name. If the remittance covers more than one person, please inform us of the name of each participant.

Payment should be in the form of:

• A bank transfer to JTB Corp. (Message: CD100720-123)

CANCELLATION

In the event of cancellation, written notification should be sent to JTB. The following cancellation fees will be deducted before refunding.
